The font is a specific, required size on both the UB 04 and CMS 1500 claim forms to allow for it to be OCR scanned (optical character recognition) in when it is processed. Making the font larger would also make the text difficult to fit into the fields.

Hi Fernando,
To print the backside (second page) be sure to open the other file in your download. There are two versions that come to you. One that prints both form and text (and the second page) and the other only the text.
When printing - if it is printing the save and print button image, you are not using Adobe Reader to print. Be sure to open the file in Adobe Reader, not Mac Preview or other PDF software.

In the UB 04 PDF put the CPT codes in the field 67 - A though Q is available for multiple codes.

You most likely have the file open with Mac preview or other PDF software. You need to use the free Adobe Reader software instead.
Steps to use:
First open the free Adobe Reader - If you need to install Adobe Reader, please use this link below.
Once Adobe Reader is installed then open the file (go to File > open > then the PDF file
Once it is open your can use the button at the top left of the form to print and save.

Hi Juan,
The machine will not need to have bold to read it. If you the human reader of this form needs better visibility, use the zoom feature within Adobe Reader so you can see the fields easier. Hope that helps!
