My UB-04 forms were delivered today, and I have yet to receive my link to the PDF's, instructions etc.

As the title says we received our forms today, but no link to the PDF's, manuals, etc.

Hi,

Glad to see you already have your paper claims.  The download files are accessed immediately after checkout, so you may have missed that. I will send the files directly to your email as an attachment.
