UB-04 CMS-1450 Not Printing Properly

Hello,

We have filled out the UB-04 CMS-1450 Form using the filler PDF provided. 


It prints properly until Section 42 (The billing lines).

It doesn't seem the line spaces for this section are accurate, everything from that section onward need to be moved down.

What can we try?
