In your download there are two versions, the one called "for text only" is most likely what you opened. I will send the version that will print the red claim and typed text to your email so you have it.

Hello! The standard font for the CMS 1500, which is used in the Fiachra Forms fillable PDF, is Times New Roman 10 point. This is universally accepted by the optical character recognition scanners. If the font is not being accepted, it is most likely that you're opening and printing the form with a software other than Adobe Reader. Please try opening Adobe Reader first, then open the form from within Adobe Reader and see if that works.

Having trouble saving
For some reason, nothing happens when i click on the pink button to save the pdf. Any suggestions for getting the drop down menu to work better?

Be sure to save it to your computer first.
Then open Adobe Reader (not other software). Then the button menu will open and you can print and save. You may have opened it in your browser or other program.

Is there a way for me to be able to edit the PDF after I have saved it? We had a billing error and I have to go back and change just 1 number on several ub04's. I'm just trying to save myself some time rather than retyping all of the information for each claim.

When I try to print on the CMS-1500 form, the printer won't print.
I'm running Mac OS Mojave. The printer icon shows up in the dock and then promptly disappears. I've deleted and reinstalled the printer. I have reset the printer to factory default. I have updated the software for the printer. Nothing seems to fix the problem. I was running the beta for Mac OS Catalina but was having too many bugs and reinstalled a back up of Mojave. That's when I started having issues.

Yes - the Fiachra Forms Fillable CMS-1500 PDF works with the latest Mac and Windows operating systems.
Something to bear in mind if you are using a Mac: Sometimes Mac computers will open PDFs in the Mac Preview program rather than Adobe Reader. To solve this issue, open Adobe Reader (a free program) and then open the fillable form from within Adobe. If you need to, please watch this video I created on how to download and install Adobe Reader for your Mac or Windows computer. You will use the free Adobe Reader every time you create a claim.
