We do have a version for the UB-04 PDF as well. It is available to you, It is not recommended however, as it may get rejected if you do not use the commercially printed UB 04 claim form.
The reason is because your office printer will not be able to print to the edge of the blank paper and parts of the red claim form will be cut off. Also we do not have the back page either so it would print only the front part of the claim. If you do want to try it to submit your claim, just send me an email to dan@fiachraforms.com and I will send you the version that will print both the red claim and typed text.
