My UB-04 forms were delivered today, and I have yet to receive my link to the PDF's, instructions etc.

Printing both sides

I purchase the form with the calculation. the more expensive one.. When I save and print, the option to print both sides is not available. It only show the print a unique pdf, or use the pre-printed 1500 forms. Also, when printing as pdf, the form shows the "save and print options" on the 1500 form. Is that meant to be that way? Thanks..

Question 21 Diagnosis field F to L limit

I can't type code, 
W27.4XXA , in  21A, 21F, 21J, 21G, 21K, 21D, 21H, 21L. Seems like the blue field is short in these. Any ideas on what can be done?

Hi  Benjamin,

Be sure you are using Adobe Reader when typing into the CMS 1500 PDF.  Then when you put in the code, type it, do not paste it in as you may have additional characters when cutting and pasting.

Diagnosis Code pointer

What format am I supposed to use for the "diagnosis code pointer" box (24E)?  It only allows four characters.  When I was submitting handwritten forms, I would always do the full diagnosis code that I entered for 21A.  For example, F43.23.  But that doesn't fit in the fillable form.  Thanks for any guidance you can provide.
