Alignment of UB 04
If the form is not aligning properly - send a scan or picture with your phone of how it prints out, and I can adjust the template to work with your printer.

UB-04 CMS-1450 Not Printing Properly
Hello,
We have filled out the UB-04 CMS-1450 Form using the filler PDF provided.
It prints properly until Section 42 (The billing lines).
It doesn't seem the line spaces for this section are accurate, everything from that section onward need to be moved down.
What can we try?
