Please can you send me a link to the PDF'S instructions. Pleaser why can't I print the down loaded form as shown on the form?

The UB 04 PDF is designed to print text only.  You will want to use it with the paper claim forms that you put in the printer.

If you an't print both form and text, you might have opened the file from the download called "for text only" which only prints the text. I will send that version to your email so that you have it.

I will be waiting please to receive the right version to my email as you had promised. If is possible I get it now I will appreciate it.

I just received my UB-04 forms, but I discovered that the text are not alining properly. How can I fix this problems.

You can send me a scan or picture with some fake information on it. Then I will adjust the template to work with your printer and send it to you for you to use. Send it to dan@fiachraforms.com
