My computer crashed and I need to reinstall the fillable UB-04 PDF

With the one-time fee, you own the fillable PDF without the need to repurchase it. Please send your name and the email account you purchased the form with to dan@fiachraforms.com and I will email you the UB-04 so you can reload it on your computer.
