Question about font in CMS 1500 PDF fillable fields

Recently a customer asked: "When entering "QK," some insurance companies are reading it as “OK” and denying claims. How do I fix this? Please advise."

First, make sure that you are opening the CMS 1500 PDF in Adobe Reader and not in Mac Preview. Mac Preview will distort some of the fonts in the form. Currently, the approved font is Times New Roman, which you can see in the filled form below. This font will be recognizable to the Optical Character Recognition (OCR) technology used by payers when they scan the form.
